The Saul and Rae Saltzman Day Treatment Centre (DTC) is a multidisciplinary Geriatric Day Hospital program for older adults living in the community with complex medical conditions who require the services and attention of at multiple clinicians (Physician/Nurse/Social Worker/Speech-Language Pathology/Physiotherapist/Occupational Therapist/) using self management principles.

Clients require a physician or Nurse Practitioner referral to attend the program, which consists of  three (3) hour sessions twice a week for up to 12 weeks. The program aims to help clients maximize their function and independence. Clients are expected to be self-motivated and active participants in both group and individual sessions, and willing to learn and follow through on recommendations. Clients who meet their goals sooner will be discharged before 12 weeks.

Program Details

Program offered: Mondays and Wednesdays OR Tuesdays and Thursdays.

  • Health Education for Patients, Families and Caregivers
  • Medical and Nursing Assessments
  • Referral to Baycrest Services (i.e. Audiology, Speech Language Therapy, Swallowing Clinic)
  • Social Work
  • Speech-Language Pathology
  • Occupational Therapy
  • Physiotherapy

Eligibility criteria

  • Participants who will benefit from four (4) or more of our disciplines (We do not accept clients for physiotherapy only)
  • Those ages 65 and over
  • Your family physician or nurse practitioner must complete and sign a referral form and maintain regular contact with you
  • Able and willing to participate in a group setting
  • Able to learn and follow recommendations delivered in English, or be accompanied by someone who can translate
  • You must have ongoing and reliable transportation to and from our program

Exclusion criteria:

  • Under the age of 65 years
  • Does not require 4 or more different clinicians
  • Requires assistance with toileting and cannot be accompanied by a support person
  • Severe cognitive impairment/mood/anxiety
  • Clients with moderate cognitive impairment that cannot be accompanied by a support person
  • Attended another day hospital within the past year and/or no new goals
