Our healthcare professionals are experts in geriatric care and use a holistic and client-centered approach. Offering a range of on-site clinic services and community and in-home programs we are helping patients and their families improve their well-being and live independently.

Adult Day Programs

 

Adult Day Programs

Adult Day Programs promote enriched living and brain health through: socialization with others; recreational opportunities; physical activities and spiritual/cultural programming. These programs offer stimulating activities for participants, and a variety of essential services and supports for caregivers. Programs include:

  • Professionally led daily exercise
  • Reminiscence, creative storytelling, discussion groups, guest lectures
  • Creative arts including painting, mixed media, ceramics, and more
  • Music programs and professionally led Glee Club
  • Intergenerational programs with local elementary/secondary schools
  • Cultural and holiday celebration programming and live entertainment
  • Specialty programs including iPad, photography, horticulture, dance
  • Outings and BBQs
  • Virtual programming also available


Eligibility: Age 65 and older (early onset dementia on a case-by-case basis). Participant programs are open to participants only, caregivers are excluded from attending.

Program hours: Monday to Thursday. 9:30 a.m. – 1:30 p.m.

Cost: There is a daily program fee and transportation may be provided at an additional cost. (Subsides are available for qualified applicants)

Alpha-Omega Geriatric Dental Center

 

Alpha-Omega Geriatric Dental Center

A healthy mouth is important at every age. Our dental care team provides a full range of services from assessments and cleaning to dentures and surgery. Our facility and expertise accommodate patients who have complex medical conditions, frailty, physical challenges or cognitive difficulties, such as dementia. Our specialized lift permits patients to receive dental treatment in a wheelchair.

Eligibility: Adults who live in the community, at Baycrest, other long-term care homes and assisted living residences.

Clinic hours: Monday to Friday 9 a.m. -4 p.m. (closed on stat holidays and Jewish holidays)

Cost: Fees are charged in accordance with the Ontario Dental Association fee guides

Behaviour Supports Ontario

 

Behaviour Supports for Seniors Program

The Behavioural Support for Seniors Program (BSSP) enhances quality of care and life for people struggling with, and demonstrating, responsive behaviours as a result of mental health, dementia, neurological conditions and brain injuries.

Partnered with local, regional and provincial agencies, BSSP helps to streamline the system for Ontario seniors, their families and caregivers who are living at home, living in long-term care homes, or in a Hospital or Transition Unit.

The BSSP is made up of specialized behavioural programs including:

  • Behavioural Supports Coordinating Office and Hotline
  • Behavioural Support Outreach Team (BSOT) in all Sectors: Community, Long-Term Care and Acute Care
  • Caregiver specialists for long-term care and community
  • Transitional behavioural support units
  • Crisis mobile services
  • Education and training


Eligibility: Older adults (and their families) who have challenging behaviours.

Cost: Fees are charged in accordance with Ontario Health

Accessibility: All referrals will be treated on a case by case basis with accessibility being considered

Hearing Services

 

Hearing Services

Hearing is key to healthy aging with research showing that hearing loss is associated with a higher risk of dementia, depression, falls and hospitalization (Reed et al, 2019)

The hearing clinic offers:

  • Comprehensive diagnostic hearing assessments
  • Hearing aid evaluations, accessories and batteries
  • Dispensing and servicing of all brands of hearing aids
  • Education and counselling for clients and their families/caregivers
  • Assistive listening devices (personal amplifiers, amplified telephones, etc.)
  • Ear mold impressions


Eligibility: Adults who live in the community, at Baycrest, other long-term care homes and assisted living residences.

Clinic hours: Monday to Friday 9 a.m. -4 p.m. (closed on stat holidays and Jewish holidays)

Cost: Hearing tests may be covered by OHIP, with a doctor referral. Additional fees are charged in accordance with technical needs.

Mental Health Services

 

Ambulatory Mental Health Clinic
and

Psychiatric Day Hospital

The Ambulatory Mental Health Clinic provides assessment, consultation, and treatment to older adults with complex medical and psychosocial issues.

Our Ambulatory Mental Health Clinic services include:

  • Assessment and diagnosis
  • Treatment on a time limited basis, including medication management (pharmacotherapy), psychotherapy, relaxation, and follow up care
  • Education and support for clients, families, and professional caregivers


The Psychiatric Day Hospital offers a multi-component, holistic approach for older adults with complex medical and psychosocial issues such as depression, anxiety and other mental health illnesses. Our team offers assessment and treatment options on an outpatient basis.

Our Psychiatric Day Hospital services include:

  • Assessment and diagnosis,
  • Treatment including medication management and non-pharmacological approaches such as psychotherapy, relaxation and breathing techniques
  • Problem solving therapy and goal setting
  • Group-based counselling/therapy
  • Group interventions/activities


Eligibility: Adults over the age of 65 who are experiencing a mental health issues such as depression, anxiety, behaviour or personality changes, confusion or significant caregiver stress. Patients must be able to attend appointments at Baycrest.

Clinic hours: Adult Mental Health Clinic - Monday to Friday 9 a.m. to 5 p.m.

Psychiatric Day Hospital – Tuesdays to Thursdays 10:30 a.m. to 2:30 p.m.

Outreach Teams: Geriatric Psychiatry Community Service (GPCS)

The GPCS provides mental health related service to individuals who are not able to access treatment onsite at Baycrest and/or those who require regular case management.

Our interprofessional services include:

  • Assessment and diagnosis,
  • Treatment including medication management and non-pharmacological approaches
  • Case management including:
    • advocating for the best possible services on behalf of clients and families
    • monitoring and ongoing support of clients
    • referrals and coordinating access to other Baycrest services and/or community programs


Eligibility: Adults over the age of 65 with a diagnosis or suspected diagnosis of depression, bipolar mood disorder, mood disorders associated with neurological conditions, mood disorders associated with bereavement, or anxiety. Patients who are unable to attend onsite appointments at Baycrest or require regular case management.

Service hours: Monday to Friday 9:00 a.m. to 5:00 p.m.

Please note: This is not an emergency service

Cost: This is an OHIP covered service

Accessibility: Appointments to be held at the patient's home.

Neuropsychology and Cognitive Health

 

Neuropsychology and Cognitive Health

The Neuropsychology and Cognitive Health program provides clinical services, education and training and clinical research related to the assessment and treatment of memory and other cognitive abilities in older adults.

Our clinical services include neuropsychological assessment, intervention, and consultation across Baycrest programs and to external organizations. External referrals are accepted for our neuropsychological intervention programs, including the following:

  1. The Memory and Aging Program serves older adults with concerns about age-normal memory changes. Self-referrals are accepted for this program. www.baycrest.org/memory
  2. Learning the Ropes for Mild Cognitive Impairment (MCI) serves clients with MCI and their family members. www.baycrest.org/ltr

Sam & Ida Ross Memory Clinic

The Sam and Ida Ross Memory Clinic offers assessment and management of challenges related to memory and cognitive health.

We welcome adults who have been referred for the following reasons:

  • Changes in memory
  • Problems with language
  • Changes in behavior


Our services include:

  • Assessment and diagnosis
  • Pharmacotherapy
  • Non-pharmacological management
  • Counselling
  • Education


Eligibility: Adults who live in the community, at Baycrest, other long-term care homes and assisted living residences in Ontario. Appointments may be in-person or virtual at the discretion of the care team in collaboration with the patient.

Virtual Behavioral Medicine

The Virtual Behavioural Medicine (VBM) Program is an escalation service for people with intractable neuro-psychiatric symptoms of dementia (BPSD), offering a rapid response where and when needed. This service is designed to be a short-term consultation service and is completely virtual, delivered using Zoom, Microsoft Teams and the Ontario Telemedicine Network (OTN).


Eligibility: Patients in acute care hospitals, long-term care homes, retirement homes and with individual community dwellings.  Patients referred will have behaviours such as physical and verbal aggression, agitation, hallucinations and paranoia and resistance to personal care.

Clinic hours: Healthcare practitioners requiring support can contact the BSO Hotline seven days a week, from 8:30 a.m. to 4:30 p.m., including weekends and holidays

Cost: No fee associated for those admitted to the program.

Geriatric General Neurology Clinic

The Baycrest Geriatric General Neurology Clinic assesses, diagnoses, and provides ongoing individualized care to patients with neurological conditions.

Individualized care includes:

  • Pharmacotherapy and non-pharmacological management
  • Counseling and education
  • Support to family members, caregivers and/or care partners.


Eligibility: Patients with diagnosed neurological conditions

Movement Disorders Clinic

The Movement Disorders clinic specializes in the comprehensive assessment, diagnosis, and ongoing personalized care for patients with Parkinson’s disease, Parkinsonism, and other movement disorders.

Services Offered:

  • Educational sessions and rehabilitation treatments tailored for individuals with Parkinson's disease and other movement disorders
  • Focus on enhancing functional abilities to address daily challenges

Spasticity Management Clinics

The Baycrest Spasticity Management Clinics specialize in providing assessment and treatment for individuals experiencing spasticity. This condition can impact those with various neurological conditions, including stroke, brain injury, multiple sclerosis, cerebral palsy, and spinal cord injury.
 


Eligibility: Patients experiencing spasticity—a condition characterized by muscle stiffness and spasms.

Specialty Geriatric and Frailty Services

 

Geriatric Assessment Centre

Our team is specialized in assessing the physical health of older adults with multiple medical and functional problems, especially those on multiple medications and/or experiencing a recent change in status with respect to mobility, memory and self-care.

Our geriatricians will be able to:

  • assess your physical and mental health
  • recommend a personalized treatment plan
  • refer you to the appropriate specialty clinics, programs and support services at Baycrest or elsewhere, as needed
  • provide recommendations to your family doctor to assist in coordinating your care plan


If you or someone you know has multiple medical problems and would benefit from a comprehensive medical, functional and psychosocial assessment, talk to your family doctor about making a medical referral to our clinic.

Saul and Rae Saltzman Day Treatment Centre (Progression and Maintenance)

Our experienced and compassionate interdisciplinary team supports adults dealing with complex health problems, supporting with medication management, pain, balance and mobility, arthritis, osteoporosis, mood disorders, memory loss, mild cognitive impairment and other conditions


Eligibility: Adults aged 65+ with complex health problems

Centre hours: Participants must commit to attending two on site sessions weekly, three hours per session for up to 12-weeks or until program goals are met. More information available with program intake.   

Cost: There is no direct charge for this program

Integrated Community Care Team (ICCT)

The Integrated Community Care Team serves homebound older adults who are dealing with complex health issues and can provide:

  • Geriatric assessment in the home, with recommendations to your referring health care provider. 
  • Home visits to share primary care with the individual’s family physician.
  • Primary care for homebound/bedbound clients who are unable to access their family doctor


Eligibility: Adults over the age of 65 with multiple medical, functional, or psychosocial problems and unable to attend appointments onsite. Clients must reside within the catchment area of Keele Street to Yonge Street and Steeles Avenue to St. Clair Avenue.

Accessibility: Assessments and visits are conducted in the client’s home

The Fall Prevention Program

The Falls Prevention and Exercise Program aims to promote falls prevention education and safety among older adults. Two programs reside within the clinic:  

  • Falls Prevention Program (10 weeks): combines education with exercise and balance activities focusing on core stability and leg strength. Topics covered include: risk factors; strategies for reaching, bending and turning; navigating your home environment and community, and fall recovery.
  • Maintenance Exercise Program: supporting clients to maintain the abilities they have gained in the Falls Prevention program.


Eligibility: Older adults aged 65 and above who have experienced a fall in the last 12 months, or have a fear of falling

Accessibility: all of our locations are wheel chair accessible

Cost: There is no direct cost for this program

Interprofessional Primary Care Team (IPCT)

The Interprofessional Primary Care Team partners with family doctors across North Toronto, to address the needs of patients aged 18 and up with chronic conditions and/or medical complexity. We provide the following services in a variety of settings based on patient needs and access, including in-home and virtually:

  • Assessment and treatment
  • Home safety assessment
  • Connecting to a primary health provider
  • Supporting transition from hospital to home/community
  • Access to supports in managing and monitoring chronic diseases
  • Connecting to free/low-cost services for those with financial barriers
  • Access to digital literacy support for virtual care and health tools.


Eligibility: Patients who are age 18+ and living in North Toronto Region, without access to primary care or interprofessional team care.

Accessibility: services are provided in-home or virtually

Seniors Support Program

The Senior Support Program offers friendly calls by trained and compassionate volunteers to support socializing and alleviate sense of loneliness as well as provide connection to information and services as needed. If individuals need more support a social worker is available for brief counseling.

Services include:

  • A friendly weekly telephone call (available in several languages)
  • A weekly email message from an ePal
  • Access to a social worker for information, referral or brief counseling upon request


Eligibility: Older adults over 65 experiencing social isolation or in need of emotional peer support

Cost: No fee required

Program Hours: Monday through Thursday from 9 a.m. to 4 p.m.

Foot Care

Our team provides professional examination and treatments to keep you active and free of pain, offering a broad range of services to promote good foot health and prevent serious foot diseases and disorders.

Our services include:

  • routine care for nails, ingrown toenails, calluses, corns, fungal infections and warts
  • pain management for heel pain, arch pain, arthritis and plantar fasciitis
  • non-surgical management of toe problems such as bunions and hammer toes
  • treatment for varicose veins


Clinic Hours: Monday, Tuesday and Thursday 9 a.m. to 4 p.m.

Eligibility: Open to all adults, and can provide specialization in diabetic care, wound care and geriatric care.

Accessibility: Services currently available on site only

Cost: Foot and wound care is not covered by OHIP, and fees for services apply.

Speech Language Services

 

FEES Swallowing Evaluations

FEES is a safe and effective instrumental assessment used in the evaluation of people with swallowing problems. At Baycrest, FEES is performed by a certified

speech-language pathologist (SLP) in consultation with an ear, nose and throat doctor (ENT) to see what is happening inside your throat when you swallow. A small tube with a camera enables them to watch and record your swallow on a computer monitor.

The SLP uses the FEES results to plan your treatment and discuss the foods and drinks which may be safer for you.


Eligibility: Open to all adults who are experiencing swallowing concerns

Lee Silverman Voice Treatment

Do you have speech problems caused by Parkinson’s disease or Parkinson’s-like disorders? Strengthen your voice and improve oral communication with LSVT® LOUD – a proven, evidence-based treatment available at Baycrest.

Treatment focuses on turning up your vocal volume, which enables you to maintain and improve your oral communication.

After treatment, 90% of patients report improved vocal loudness and all patients report improvements in their ability to communicate.


Eligibility: Adults of all ages who have Parkinson’s or other neurological disorders

Clinic hours: Participants will be required to attend a one-hour session four days a week for four consecutive weeks

Accessibility: Accessibility to be assessed during referral review

Cost: This service is not covered by OHIP. A fee for service applies payable upon service provision
